Instructions

Once you have added a form/resource (item) to your department or college eforms page, please forward this information to update the eforms A-Z index to Web Services.

New item to be added (complete sections A and C below)Revise existing item (complete sections B and C below)

Section B: For revisions

Does the name of your item change?

If yes, what is the old item as it appears on the eforms A-Z index

And, what is the new name as you would like it to appear

Does the file name change? (Remember, it is always best to overwrite a previous file with the revised file whenever possible to prevent broken links that may reference your form from other pages on the web.)
