Paying for Your Insurance Benefits

If you are in an unpaid status long enough to not have sufficient pay for the deduction of your insurance benefits premium, in order to maintain continuity of coverage, it is your responsibility to ensure that premiums are paid. Premium payments are due no later than the 25th of the month to maintain the next month's coverage.  To pay online please click on the box below that matches your place of employment. If you are on a leave of absence, to ensure proper credit on your account, please make sure to confirm that your remittance amount matches the amount due stated on your leave of absence letter. Premium payments will continue to be due and payable on the 25th of each subsequent month until your return to active employment.  

Failure to pay premiums will result in your coverage(s) being canceled. Once health insurance has been terminated, any and all medical, dental and/or prescription claims, from the date of termination, become the sole responsibility of the employee.

If you allow your benefits to lapse, upon return from leave you will need to request reinstatement  of coverage. You will have 30 days from your return to work date to request reinstatement of coverage. The effective date of coverage will be the first of the month following your return to work date. Please contact your Human Resources office directly for instructions on how to reinstate your coverage.
